The United States Department of Education requires our office to collect certain information from students and parents to make sure that we provide you with the best available award package.

Form and Document Submission Guidelines

For most forms, the financial aid offices uses an electronic form submission process called Dynamic Forms which is accessed through the Financial Aid Dashboard. Please see below for form and document submission guidelines. 

All tax information including W-2's, schedules, tax transcripts, and tax returns must be submitted through the requested form on Self Service Carolina. If you are submitting a federal tax return, a signature on the tax return is required (handwritten). Handwritten Signature(s): Typed/digital/electronic signatures are not accepted. You must download and sign the tax return.

  • Include Student's Name and VIP ID on all documents.

  • Personally Identifiable Information (SSN, Date of Birth) must be redacted or removed unless specifically requested by the financial aid office to continue processing your aid.

  • Forms for verification, loans, or satisfactory academic progress that are unable to be submitted electronically must be submitted by fax, email, mail or in person.

  • Please upload one PDF or PNG file for every item requested. We cannot accept .MOV files. 

  • Files over 25MB will not be accepted.

  • We cannot accept compressed, zipped, password protected files, and/or login credentials to an FTP site.

  • Important! We are not able to accept HEIC file formatted documents.

  • Verify that all documents are legible and the appropriate signatures (handwritten) and dates are included.

If you submit a file type that is not listed, your document will be rejected and you will be required to submit an acceptable file type that can be opened by the Office of Student Financial Aid and Scholarships.
