Instructions for Updates
Please read all of these instructions before continuing to the next section.
Please include all below information regarding members whose status needs to be altered on the FSL roster below. You may submit up to 10 status changes/form:
- USC or VIP ID is required to make member change.
- Documentation (Signed Academic Release and Signed Anti-Hazing Pledge) is required to add members.
- New members in NPC groups joining outside of structured recruitment must also sign a COB MRABA form.
- All forms can be found under "Forms and Applications" on the FSL Website.
Please note, organizations do not need to change new members (P) to active members (A) through this form. That is automatically done after grades are pulled each semester.
(P) = add new member to roster outside of Formal Recruitment/Intake
(A) = add initiated member or affiliate member to roster
(I) = remove member due to resignation or removal
(G) = remove member due to graduation
