Chapter 35: Survivors and Dependents Education Assistance Program Checklist

 

First Time Applicants

Certification sent to Muskogee VA Regional Processing Office through VA Once:

  • After the veteran is formally accepted to the University
  • Up to 120 days prior to the 1st day of class

Required Documentation

  • VONAPP or VA Application Form 22-5490
  • Copy of Birth Certificate with both parent's names (optional)
  • Disability Rating Certificate (optional) or Line of Duty (if deceased-optional)
  • Enrollment Certification Request form
  • Note: A “Certificate of Eligibility” from the VA can substitute for the VA form 22-5490 and the birth certificate.

Transferring Benefit from another School

Certification sent to Muskogee VA Regional Processing Office through VA Once:

  • After the veteran is formally accepted to the University
  • Up to 120 days prior to the 1st day of class
