Documentation

The accommodations granted will depend on the documentation submitted. This includes its support of the diagnosis and how it meets the definition of a person with a disability under the ADA.

What is accepted as documentation?

You must submit documentation from a current treating physician that:

  • is on letterhead;
  • is signed and dated;
  • states a diagnosis or diagnoses;
  • provides support for the diagnosis and how it meets the definition of a person with a disability under the ADA.

Information about IEP and 504 plans and the information USC will consider when evaluating requests for accommodations.

Performance Summaries from High School

What it is. USC does not accept these as documentation of a disability.
