Instructions and Information for Completing the Federal Perkins, Nursing, and Health Profession Master Promissory Note, Indebtedness Statement, and Requried Entrance and Exit Counseling
Once the Office of Financial Aid awards you a Federal Perkins, Nursing or Health Profession Loan, you are required to complete the following before receiving any loan proceeds:
- Promissory Note
- Indebtedness Statement
- Entrance Loan Counseling
- Self Certification Form (Only for Nursing and Health Profession Loans)
THE FEDERAL PERKINS, NURSING, AND HEALTH PROFESSION LOAN
Loan paperwork is available on the Bursar's web site in the PDF format. In order to view and print them, you need to use a free product called Adobe Acrobat Reader. It is available for download at http://www.adobe.com/products/acrobat/readstep.html. We suggest using the current version. Once you have completed the paperwork, you must bring or mail it to the Bursar's Office in order to receive your loan funds. The promissory note will also be mailed, as some students may not have computer equipment available.

The paperwork you need to complete is:
- Federal Perkins Master Promissory Note (PDF)

- Nursing and Health Profession Indebtedness Statement (PDF)

Steps to Completing the Perkins Paperwork

All items must be completed legibly in ink or typed

Make sure you include which state issued your driver's license in item 5.

If you leave any part of the Promissory Note blank, your loan funds will not be processed until it is complete.

The document must be signed and dated in ink.

Items 1-5 must be completed.

The Lender Copy and File Copy (both are marked page 1 of 4) must be returned to:

Please retain the copy of the Promissory Note marked "Borrower" and the "Terms and Conditions".

Please complete all information requested on the Indebtedness Statement. If you do not know the amount of funds you have received to date please call (803) 777-4077.

Mail Indebtedness Statement along with the Promissory Note.

Students receiving Nursing and Health Profession Loans must also complete and mail the Private Education Loan Self Certification form.

You may also mail your Entrance Loan Counseling confirmation page.
- Students may complete Entrance Loan Counseling or Entrance Interview at http://mappingyourfuture.org/OSLC/.
Please remember your loan will not be released until you complete all necessary paperwork and loan counseling.
