I. Policy
The Office of Communications in Computer Services is responsible
for modifying telephone services and equipment.
II. Procedure
A. Send requests by memo for adding, moving,
disconnecting or other modifications to any type of
telephone service and equipment to the Office of
Communications.
B. Lead Time
1. Submit requests for software changes at least two
weeks before desired due date.
2. Submit requests requiring physical relocation of
equipment at least three to four full work weeks before
desired due date.
3. Submit complex requests such as departmental moves
or the physical relocation of ten or more sets at least
two months before desired due date. Floor plans of new
areas are required.
4. Ambiguous or incomplete information in requests
will create delays. Consultation with Communications
personnel is encouraged and readily available.
C. A letter confirming receipt of each request, an
estimate of charges and an approximate due date will be sent
to the contact person named in the memo.
D. Charges
1. The requesting department will be charged for all
monetary expenses incurred by Communications in
completing any request.
2. The department using the service will pay any
monthly charges from external vendors.
3. Exceptions must be approved by the Vice President
for Computing and Communications.
E. Area Campuses
1. Process requests through the appropriate personnel
at each campus.
2. For modifications to equipment other than the
System-85 telephone system, submit a memorandum stating
the following:
a. department/Campus name and phone number;
b. contact person;
c. thorough description of request;
d. desired date of completion.
