  New form templates were created in 2009 to streamline the process for candidates when preparing their files. Previous Phase I or Phase II versions can no longer be used for submission of materials related to either the Third Year or the Tenure and Promotion review process.

Submission Process
The Office of the Provost, in collaboration with the University Committee on Tenure and Promotion, has developed an electronic submission process. Colleges and schools will submit files electronically, in PDF format.
