• Scan front and back of any previous EAD (if you had any) 
• Scan of previous I-20s showing CPT or OPT (if applicable) 
• Digital I-20 with OPT recommendation:
  • For post-completion OPT: the I-20 must be issued within the last 30 days. 
  • For STEM OPT extension: the I-20 must be within 60 days. 
  • The I-20 must be signed by both the ISS advisor and you (can be digital or handwritten) 
• Your most recent Form I-94 
• Credit card to pay the filing fee

• Indicate your eligibility category (e.g. C3B for post-completion OPT) 
• Fill in personal information such as name, contact info, U.S. address, date and place of birth, passport and visa details, SEVIS ID, etc.
• If you have filed Form I-765 before, answer accordingly and provide evidence

• 2×2 photo
• I-94 record
• Previous EAD (if any)
• I-20 (with OPT recommendation)
• Previous I-20s with CPT/OPT (if applicable) 
• Important: Do not upload a previous I-20 lacking OPT recommendation. Only use the I-20 issued by ISS for OPT, per guidelines. 
• If missing any prior I-20s, use the “Additional Information” section to explain prior training (CPT/OPT) with dates, status, etc.

• Carefully review all fields
• Agree to the applicant statement
• Digitally sign
• Pay the filing fee
• Submit your Form I-765 application online

• Eligibility category: C3B 
• Select “Initial permission to accept employment” as your reason

• Eligibility category: C3C
• Indicate your STEM-eligible degree and employer’s E-Verify name & number (5–7 digit)
• Reason for applying: “Initial permission to accept employment” for extension
• Upload I-20 with STEM OPT recommendation (issued by ISS) 
• Upload diploma or proof of STEM degree (if required) 
• If applying based on a prior degree (not your most recent), upload accrediting institution documentation
