Please include a statement a regarding accessibility of course material in the Provisions for Course Content Interactions section of the proposal. Also, please add a statement regarding technology and minimal technology skills required in the syllabus. Here are examples you can modify and use:
Accessibility Statement Example 1The professor will meet with an instructional designer at the Center for Teaching Excellence (or department/college ID staff) to ensure that the course materials posted on Blackboard incorporate basic accessibility features. The course syllabus includes an accessibility statement that encourages students with disabilities to register with the Office of Student Disability services; should a student with a registered disability enroll in the course, the professor will work with the Office to make any additional accommodations appropriate to that student’s needs.
Accessibility Statement Example 2All documents including syllabus and description of assignments will be provided in both accessible Word and PDF formats. For instance, documents will have proper headings, alternative text, and properly labeled links where necessary. Likewise, PowerPoint slides will be made accessible by providing alternative text where necessary, ordering textboxes and other recommended ways of making PPTs accessible. Recorded lectures (if a part of the course) will be accompanied with transcripts in accessible Word and PDF formats.
Technology and minimal technology skills required statement ExampleStudents are expected to have basic computer skills including using the Internet, specifically, the Web. They are expected to log into the course site at least once a day. Students should be able to interact with the course site and resources in Blackboard (posting to Blackboard discussion board, submitting assignments, etc). Students will be expected to use create wikis in Blackboard and prepare reports using a word processor.
The minimal technical skills required include the ability to:• organize and save electronic files• find and use web resources (e.g. external resources for the wikis)• use email and attach files, • check email and Blackboard daily, and • download and upload documents via email and Blackboard.• create a recorded presentation in MS PowerPoint

This course is proposed as 100% synchronous. If so, when and for how long will sessions be delivered? This is an issue for distributed learning classes. So, specify here or account for learning minutes and student-student, student-content, and student-instructor contact.
Also, the "course expectations" don't make a lot of sense (they appear to have been written for a face-to-face class).
You are welcome to look at the Committee for Instructional Development web site for additional examples of distributed learning requirements.
Make sure the course expectations are consistent with a synchronous course syllabus. That needs to be addressed. There are resources on campus to assist with this.
For the sake of cosmetics, the double numbering of the learning outcomes might be fixed.
