The form below can be used to submit either an individual faculty activity report or a chair/director activity report. A complete description of the three review process types and required steps can be found in the full Appointment and Review of Department Chairs and Directors policy.

Individual Faculty Activity Reports
All department chairs, and any directors not housed in a tenure-granting unit, should use the form below to submit a two-page summary of research, teaching and service activities and achievements as an individual faculty member for the 2019 calendar year to the Office of the Dean.
(Program and center/institute directors with tenure homes will be reviewed by the chairs of their home units in accordance with usual procedures.)

Chair/Director COMPREHENSIVE Review Process
Department chairs and directors in their final year of appointment who wish to be considered for reappointment, and those directed by the Dean, should use the form below to submit a two-page summary of activities and achievements as chair or director.
