There are two types of NIH budget forms:
- Research & Related (R&R) Budget Form: If applying for more than $250,000 in direct costs per year, the R&R budget should be used unless the program announcement states otherwise. NIH funding announcements that require other forms will include a link to download the appropriate Adobe Forms package from Grants.gov. (For proposals created in NIH ASSIST, the budget is entered directly into that system.) Download a sample Research & Related NIH Budget Form [pdf].
- NIH PHS 398 modular forms: Download a sample NIH PHS 398 Modular Form [pdf].

The NSF requires that investigators enter budgets directly into the FastLane proposal system. Refer to the NSF FastLane Budget Sample [pdf] and the NSF FastLane Budget Sample with Subaward [pdf] to preview the FastLane budget documentation requirements.
The generic budget template [xlsx] may be used to create a budget for a sponsored project lasting one to five years. The sponsor's budget form or proposal system may vary. Many sponsors require that the budget be entered into the sponsor's proposal system. This should be addressed in the program guidelines and/or agency policy manual.
