Apply for Assistance
Submitting an application (Form Coming Soon) as soon as possible allows the coordinator of temporary conditions and injuries to
connect you with resources and begin working on your behalf.
Your application should include documentation from a qualified medical professional about the nature of your condition or injury and how long you will need assistance. Once your application has been reviewed, you will be notified by phone or email about your status.

- Extended time for testing
- Breaks during testing
- Use of technology for note-taking, including the use of a cell phone
Communicate With Your Professors
Even if you don’t have immediate access to documentation, submit your application(Form Coming Soon) and email your professors to let them know that you have started the process to register regarding your temporary condition or injury. You don’t need to share detailed medical information, just enough so that your professors understand why you might need assistance.
