USC Brain Health addresses disparities in cognitive care by collaborating with health systems statewide. Together we are building an expansive network to provide coordinated care for South Carolinians experiencing cognitive concerns, including changes in memory, thinking and personality.   

USC Brain Health Network

The Brain Health Network is a state-funded initiative dedicated to identifying and addressing the growing need for early detection and management of Alzheimer’s disease and related dementias (ADRD) and mild cognitive impairment across South Carolina.

The Brain Health Network supports primary care providers by performing comprehensive cognitive screenings and evaluations that improve access to specialty care services at the primary care level. 

Our model includes a 90-minute cognitive screening performed by a Speech-Language Pathologist, designed to identify early Alzheimer’s disease, related dementias and mild cognitive impairment.

Results are evaluated by an Advanced Practice Provider who determines appropriate resources which may include a referral to neurology or other appropriate specialty services and therapies. All options are thoroughly discussed with the patient and their caregiver. 

A Community Health Worker is available to share community-based resources and provide ongoing support. 

Our approach to a collaborative care model reduces unnecessary referrals, accelerates care for patients needing advanced screenings and provides referring providers with clear recommendations for care management.
