How to be seen at a USC Brain Health Network clinic

If you or your family have concerns about changes in memory or thinking, the first step is to talk to your primary care provider. Your healthcare provider can determine if a referral to the USC Brain Health Network is appropriate.

If you are referred to the USC Brain Health Network, our team will complete a thorough cognitive and functional assessment to better understand your concerns and current level of change.

After your screenings and evaluation, we will develop a personalized care plan to meet you and your care partner or family's needs and goals. The care plan is also shared with your referring provider, who remains responsible for your ongoing care.

The Appointment Process

We schedule two Brain Health Network appointments with the patient and/or the caregiver to determine care plan recommendations. These results are shared with your referring provider to manage your care or provide next steps for you.

Your first appointment will be an in-person visit with a Speech-Language Pathologist. They will talk with you about memory, thinking, daily activities, your concerns, and will give you the cognitive screening.

Your second appointment will be an in-person visit with a health care provider. They will have evaluated your screening results and discuss level of cognitive change and provide individualized recommendation and education to help you understand your next steps.

When both appointments are complete, our team will send your screening results and recommended care plan to your provider.
