Chapter Program Submission

This form gets turned in to submit any chapter programs for ACE points. This form will be due on November 20th by 11:59 PM.

End of Semester Form

Chapter presidents will fill out this form at the end of every semester. This form will be due on April 28th by 11:59PM PM

Grade Release

Only chapter presidents or chapter advisors may complete the following form to request access to the chapter’s semester grade report.

FERPA

Chapter presidents and chapter advisors must complete this form once a calendar year in order to be able to access chapter grade reports

Roster System Access Request

Please submit this form if your chapter needs to change which officers have access to the roster system. As a reminder, only two chapter officers can be given access to the roster system.

Update Chapter Roster

Two chapter officers may use this site to add new members to the chapter’s roster, as well as remove them, mark them as graduated, or as studying abroad.

Advertising Request

Submit your ad request by Friday at 5 p.m. to appear in Wednesday's newsletter.

Banner and Decorations Form

Submit this form at least a week before your banner or decorations are hung. Banners are for philanthropic, service, school spirit or recruitment events, not social functions.

Event Notification Form

Submit your form at least 7 days before your event.

Incident Report

Report a conduct violation online. Choose a Greek Life category under "Nature of this report."

Panhellenic COB MRABA

Members who join a CPA organization outside of the formal recruitment process must complete this form.

Initiation Confirmation Form

This form must be filled out within 24 hours of initiation being completed. 

Affiliate Background Check Request

Property Managers should use this site to request a background check.

Property Manager Leave Request

Staff should use this site to request leave.

Property Manager Facility Update Report

Property Manager Facility Update Reports are due by communicated deadlines from the supervision team.
