Passport Application Guide

Use the guide below to learn how to apply for a passport and what documents are needed.

Step 1: Plan to Apply Early

As processing times may vary, it is important to apply early. You must apply for a passport book, not a passport card.

Step 2: Prepare Required Documents

  1. Passport Application Form, unsigned
    • This form can be filled out online and printed, or printed and filled out by hand
    • Do not sign this form until told to do so at your appointment
    • Form must be printed single-sided
  2. Proof of US citizenship and a black and white photocopy
    • Acceptable documents:
      • US birth certificate
      • Consular report of birth abroad or certification of birth
      • Certificate of naturalization
      • Certificate of citizenship
  3. Original photo ID- must bring the original and a black and white photocopy
    • Acceptable documents:
      • In-state driver’s license or enhanced driver's license
        • You may need to bring a second photo ID if your driver’s license is from a different state
      • Military identification card
      • Current valid foreign passport
      • Trusted Traveler ID (ex: Global Entry)
      • US Permanent Resident Card
  4. Passport photo-  must meet specific guidelines
  5. Passport fee- by check or money order only  

Step 3: Apply In-Person

  • Find a Passport Acceptance Facility and schedule an appointment if required
    • The USC Post Office at 1600 Hampton Street serves an on-campus Passport Acceptance Facility.  See the USC Post Office website for more information about our on-campus facility.
  • Bring all documents listed
    1. Passport Application Form, unsigned
    2. Original proof of US citizenship and photocopy
    3. Photo ID and photocopy
    4. Passport photo
    5. Passport Fee
