Step 1: Plan to Apply Early
As processing times may vary, it is important to apply early. You must apply for a passport book, not a passport card.
Step 2: Prepare Required Documents
- Passport Application Form, unsigned
- This form can be filled out online and printed, or printed and filled out by hand
- Do not sign this form until told to do so at your appointment
- Form must be printed single-sided
- Proof of US citizenship and a black and white photocopy
- Acceptable documents:
- US birth certificate
- Consular report of birth abroad or certification of birth
- Certificate of naturalization
- Certificate of citizenship
- Acceptable documents:
- Original photo ID- must bring the original and a black and white photocopy

Step 3: Apply In-Person
- Find a Passport Acceptance Facility and schedule an appointment if required
