Forms and applications for fraternities and sororities.
Only chapter presidents or chapter advisors may complete the following form to request access to the chapter’s semester grade report.
For the ACE Program, we ask chapters complete the Kognito At-Risk for Students Module. Chapters should have 10% of their membership complete the module; we recommend this be chapter officers and chair positions, but it can be anyone in the chapter. Please note, since incoming freshmen are required to complete the training, they do not count towards the chapter expectation. Each member should use this form to upload their completion certificate.
Please submit this form if your chapter needs to change which officers have access to the roster system. As a reminder, only two chapter officers can be given access to the roster system.
Two chapter officers may use this site to add new members to the chapter’s roster, as well as remove them, mark them as graduated, or as studying abroad.
Submit your form at least 7 days before your event.
Submit your guest list up to 24 hours prior to the event.
Submit your form confirming chapter attendance at a presentation on harm reduction and risk management.
File this form during the first week of classes to certify your compliance with the anti-hazing policy. Updated form coming soon. Please email Tad Derrick for more information.
Report a conduct violation online. Choose a Greek Life category under "Nature of this report."
All new members must complete this form prior to being added to their chapter's official roster.
Potential members must submit this form by bid day.

The Office of Fraternity and Sorority Life is proud to offer scholarships for members of our fraternity and sorority community to attend a session of the Undergraduate Interfraternity Institute (UIFI) or sponsor them to attend their national organization’s events/conferences or another national leadership conference. These scholarships can cover cost of attendance and travel to these immersive learning experiences, but will not include hotel accommodations or meals (unless covered in the registration cost).
Undergraduate Interfraternity Institute (UIFI), hosted by the Association of Fraternal Leadership and Values
At UIFI, students learn with and from peers who belong to various organizations and councils at schools from across the country. Through exploration of critical issues in thought-provoking activities, the program is intended to help identify the leader you want to be and where you can make the most impact in your chapter, council or community. Together you’ll define the challenges that our communities face today, and leave excited to address those challenges back on campus – all while developing skills that are useful in and out of fraternity and sorority life. Multiple UIFI sessions are scheduled throughout each spring and summer. UIFI is hosted at chapter houses at the IU-Bloomington campus. Registration covers the cost of lodging and some meals.
Please note that this application does not guarantee registration or travel fees being paid. Someone from the Office of Fraternity and Sorority Life staff will be in contact with you shortly to let you know if your application has been accepted. We recommend you do not begin making travel arrangements until receiving confirmation.
This year's application has closed. If funds are available next year, an application link will be made available in the early spring semester.
