Overseas Insurance Form-Business Travel

As a benefit, any UofSC faculty or staff member going overseas on University sponsored business that is unrelated to study abroad have the option of being enrolled in a specialized emergency medical insurance plan for the duration of their time abroad.

University sponsored business includes overseas-based activities such as: conducting research, presenting at a conference, etc. Spouses/partners of UofSC faculty or staff are not eligible. Insurance enrollment is required when leading a group of students (i.e. Global Classroom) abroad. If you are traveling in connection to study abroad programming, please contact the Study Abroad Office.

Insurance coverage is provided by Cultural Insurance Services International. This plan is not intended to take the place of one’s primary health insurance coverage. It is only supplemental and provides coverage specific to overseas travel.

The charges for this coverage are calculated in weekly increments at the rate of $15.75/week (as of 8/22/2019). The monthly rate for coverage is $63. Please consult with the Study Abroad Office regarding calculation of charges. Employees may elect to pay for this coverage themselves, or an individual’s department may decide to cover the cost. The information requested below will be used by the UofSC Study Abroad Office to enroll individuals in the insurance plan and bill them or their department, as specified.

This form must be completed and submitted to the Study Abroad Office at least 30 days prior to departure. Individuals who do not submit this paperwork according to these guidelines will not be enrolled.
