Grade Release Statement

All members sign a membership card when they join the FSL community which allows the Office of Fraternity and Sorority Life to release academic information to the following individuals only: 1) chapter president, 2) chapter advisors, 3) intern/national headquarters staff, and 4) Office of Fraternity and Sorority Life staff. Please do not submit this form if you do not meet one of those criteria. The statement of release of academic information is for the following reasons: 1) verifying eligibility for recruitment/intake and continued membership, 2) awards recognition, 3) verifying eligibility for officer roles, 4) verifying eligibility for Honor Society membership, and 5) notification of any conduct or academic integrity matters which may be a matter of health and safety.
