Transcript requests are processed by the Office of the University Registrar which
is located on the main campus. The current fee for official transcripts is $12.00.
Transcript requests can be submitted online via the Office of the University Registrar's webpage. Requests are typically processed 2-3 days after receipt.
Student vaccination records are kept on file for 20 years. Requests to obtain a copy
can be sent to Jennifer Evans, Student Health Nurse, via email or at (803) 216-3374. You must be able to provide a full name, DOB, and SOM enrollment
dates for verification.
Verification of Medical Education
Verification of medical education requests can be sent to firstname.lastname@example.org
Fellowship applications require the submission of your medical school transcript and Medical School Performance Evaluation (Dean’s Letter).
You may request these documents through your online application. The request will be received by the School of Medicine and uploaded to your application through the Medical Institution Document Upload Service (MIDUS). Please use the information listed below to submit your requests:
