This study is being conducted by Dr. Delia West in the Department of Exercise Science, Arnold School of Public Health at the University of South Carolina.
The purpose of this study is to look at the physical activity, sedentary behavior (sitting time), eating habits, stress levels, and sleep patterns of undergraduate students at the University of South Carolina. Study participation requires completing two surveys focused on your typical lifestyle behaviors, one now (at the beginning of the semester) and a second one later in the semester to see whether things might have changed. The survey is brief, taking up to 20-30 minutes to complete.
We will survey up to 400 USC undergraduates in this study. Should you decide to participate, once you complete the first survey, you will be given the opportunity to be entered in a raffle for an incentive (an Echo dot, Fitbit or other incentive) to show our appreciation for your time and effort. We will hold 4 different raffles (the first for the first 100 students who complete the survey and the second raffle for the next 100 students who complete the survey and so on). Therefore, you will have at least a 1 in 100 chance of being the winner of the raffle.
The follow-up survey will be very similar to today's survey. An email with a unique, personal link to the questionnaire will be sent to you later in the semester (before the end of classes and before final exams). If you complete that second survey, you will receive a $10 financial incentive (electronic gift card) for completing the second survey. Once you complete the follow-up questionnaire, you will also be given the chance to be entered in another raffle for an incentive. Once again, there will be four raffles for incentive prizes and you will have at least a 1 in 100 chance of being the winner of the raffle.
If you are participating in this study to satisfy a research requirement for a class, you will be asked at the end of the second survey to provide your name, your instructor’s name and your class number (department designator, course number and section number) so that research staff can let your instructor know that you participated. Your instructor will be alerted that you completed the study but will not be provided with any of the information you reported on the survey. Your personal information will remain confidential and will not be shared. Only research staff will have access to the password-protected survey data. Additionally, all data from the study will be presented at scientific meetings and in publications in aggregate, with group data reported and no way to identify individual information.
To be eligible to participate in this study, you must be an undergraduate student at the University of South Carolina.

Should you decide that you wish to withdraw from the study before you have completed both surveys, you can contact Dr. West and let her know. Data you have provided up until that point will be retained but you will no longer be contacted by research staff.
