Please print the following document. Review, complete the top portion, and deliver
a signed paper or scanned form to each letter writer before the letter is submitted.
This document must be submitted by each of your letter writers along with his/her
letter of recommendation.
FOR LETTER WRITERS:
Please submit your letter of recommendation AND the student’s signed waiver form for
his/her application to the USC School of Medicine Greenville Early Acceptance Program.
The letter must be on letterhead and signed. The deadline to submit is 5:00 pm, December
1, 2018.
