I agree to the terms below (on behalf of a minor)I authorize the University of South Carolina to make use of photographs or video taken of my child/minor, as well as personal quotes or statements obtained through interviews or audio recording for any official University purpose including marketing and promoting the University of South Carolina and its programs. I further grant to the University of South Carolina the perpetual right to any use of the photographs or video taken of my child/minor and any quotes or audio recordings attributed to them, either solicited or unsolicited, at its discretion, whether through its own facilities or those of other agencies or organizations to whom it subsequently grants use. I also agree that the University of South Carolina shall not be required to include any credit identifying any persons in the use of the photographs, video, written quotes, or audio clips for certain purposes. I have not received payment nor been promised anything of value for use of my child/minor’s image or statements. I do hereby release the University of South Carolina, its successors and assigns, from any and all claims for damages, for libel, slander, invasion of privacy, appropriation of my child/minor’s image or personality, or any other claims based upon the use of the photographs or video taken of my child/minor and any quotes or audio recordings attributed to them.
