One of the most important things you can do to get ready for Graduate School is to complete your immunization requirements and upload needed documentation to the My Health Space portal.
Immunization Requirements
The University of South Carolina complies with federal health regulations requiring proof of immunity to Measles, Mumps and Rubella and a completed TB Screen Risk Form from all students. Refer to the Student Health and Well Being's Immunization page and frequently asked questions page for more information about required immunizations and possible exemptions.
Failure to comply with immunization requirements early in your first semester will
result in a registration hold that will remain until resolved.
How do I submit proof of immunity?
You will submit your immunizations information on MyHealthSpace under "Medical Clearances." Step-by-step instructions with screenshots are provided on the Submit Your Immunization Forms page (scroll past the undergraduate orientation info).
- Take a picture or scan your document(s)
- Log into: Https://myhealthspace.ushs.sc.edu/login_dualauthentication.aspx
- Click on Student or Faculty and Staff
- Login using your network username and password.
*Network username is assigned by the university and is the first portion of your university email address.
- Click Proceed
- Enter your date of birth (MM/DD/YYYY)
- Click Proceed
- Click Medical Clearances (left side of the screen)
- Click Update next to Immunization Record
- Click Upload and add your document.
- Scroll to the bottom of the page and Click Save or your document will not upload.
- Click Update next to each
- Add the date of immunization under Date 1.
- Select type of vaccine under Vaccine 1 by clicking the downward arrow on the right. (This must be completed for each vaccine).
- If adding Titers enter date titers were drawn under Date and results by selecting Negative or Positive. (This must be completed for each Titer and lab results uploaded).
- Click TB Screen Risk Form
- Click the Update button and answer each question.
- Click Submit
*If tuberculosis testing is required, the system will automatically prompt you after completing the TB Screen Risk Form. To fulfill this requirement, you must have either a QuaniFeron Gold or T-Spot blood test performed within the past one (1) year, if positive, a Chest X-ray is needed that has been performed within the past three (3) months. All documents must be Translated into English by your professional health care provider.
Students who are coded in a fully online program will have a "Distributed Graduate Education" concentration showing in your my.sc.edu profile. These students will automatically show in MyHealthSpace as being immunization compliant.
Those who are participating in coursework at another location or are choosing to take only online classes will need to submit proof of immunization.
You can find a Distance Learner Immunization Exemption Form (Non-Degree students only), a Medical Condition Immunization Form and a Religious Immunization Exemption Form on MyHealthSpace under "Downloadable Forms." Upload buttons are provided on the forms page.
